  2. Complete the Youth Application Form.

  3. Complete the Medical Form (Parts A&B).

  4. Print and Sign Each

ADULT APPLICATION

  1. Complete the Adult Application.

  2. Print it and sign it. 

  3. Email it to Pack Committee Chairperson Jennifer Champion at secretary@champion.net

  4. Complete your YPT (Youth Protection Training), your application cannot be processed without the completion of your YPT.
